To make an appointment to meet with a member of the Survivor Care Team, please do one of the following:

Note: SAPAC is open Monday through Friday from 9 am - 5 pm. Please share your availability for within that time frame.
Same-Day Appointments may be available upon request. Please call or email to check for same-day availability.
Walk-Ins: You are welcome to walk in to the SAPAC office and request to speak with a member of the Survivor Care Team. Depending on availability, our administrative assistant will either connect you with the Survivor Care Team on-call person at that time, or schedule an appointment for you and the on-call person’s earliest joint availability. During times of COVID-19, we strongly encourage anyone seeking an appointment to call or email first.
The administrative assistant will confirm the date and time of the appointment, as well as the member of the Survivor Care Team with whom you’ll meet. They will then send you intake paperwork to complete before your appointment, and you’ll receive a meeting invite.
