To make an appointment to meet with a member of the Survivor Care Team, please submit this service request form.
Our Intake Coordiantor will email after we recive a request to confirm the date and time of the appointment, as well as the member of the Survivor Care Team with whom you’ll meet. They will then send you intake paperwork to complete before your appointment, and you’ll receive a meeting invite.
Same-Day Appointments may be available upon request. Please call or email to check for same-day availability.
Walk-Ins: You are welcome to walk in to the SAPAC office and request to speak with a member of the Survivor Care Team. Depending on availability, we will either connect you with the Survivor Care Team at that time or work to get you set up with an appointment for another time. We strongly encourage anyone seeking an appointment to fill out the request form first.
